Study Description: Obesity, a growing epidemic for US and worldwide transplant recipients and a health care priority in Healthy People 2010, is a risk factor for type 2 diabetes mellitus, hypertension and other chronic diseases in renal transplant recipients. Renal transplant recipients experience an average weight gain of 22 lbs (10 kg) during the first year following transplant, which is significantly more than the 2 lbs (~0.9 kg) average yearly weight gain in US adults. The rate of obesity in renal transplant recipients has increased and is associated with significantly higher morbidity and mortality. In addition, African American (AA) females are most at risk for weight gain and morbidity. This predictable and significant weight gain within a short amount of time, particularly in AAs, and its association with morbidity and mortality make this a high priority concern. Advances in immunosuppressant therapies have resulted in improved graft outcomes. Yet, these same advances combined with an improved appetite, place renal transplant recipients at risk for obesity. Cutting edge microarray methodologies in combination with environmental data and Bayesian network modeling will allow the identification of key environmental risk factors and gene expression profiles (indicating potential susceptibility genes) associated with obesity. Importantly, these data could be used to identify those at risk for obesity allowing us to target intensive preventive treatments to modify environmental factors within the context of an individual‘s genetic framework. The purpose of this study is to prospectively examine key environmental risk factors and gene expression profiles contributing to obesity at one year following renal transplant in recipients whose BMI was < 30 at time of transplant (non obese). We are excluding transplant recipients who are obese at time of transplant so that we can isolate changes in gene expression levels associated with obesity.
